HTA publishes revised guidance on reporting incidents in the post mortem sector.
The Human Tissue Authority (HTA) has issued an update for establishments operating under a post-mortem license about the submission and completion of HTA Reportable Incidents (HTARI). This update follows a comprehensive internal review prompted by an increase in both the severity and frequency of reported incidents.
The revised guidelines aim to ensure establishments are informed of the updated protocols and are equipped to adjust their policies and procedures accordingly. Key actions implemented by the HTA include the addition of a clear definition of reportable incidents, ensuring Corporate Licence Holder contacts are included in initial communication and incident closure notices, and the potential sharing of incident information with other regulatory bodies. Additionally, establishments are provided with guidance on completing incident summaries, emphasising immediate corrective actions, and are supported with a Decision Tree to navigate incidents involving Accidental Damage during post-mortem Post Mortem Examination.
Establishments are urged to promptly review their Standard Operating Procedures (SOPs) and policies to align with the updated guidance. Furthermore, dissemination of the revised guidelines to all staff and designated individuals involved in licensed activities is mandated, with instructions to remove any previous hard copies from circulation.
